As of October 2011 The MotHERS Program has provided the Post Partum Health Record to every woman who gives birth at Kingston General Hospital.

By filling out this survey you will be helping us to improve the Postpartum Health Record and provide better resources to new moms in our community!

The Postpartum Health Record is a tool to help new moms and their doctors monitor weight, blood pressure and activity level changes for one year following delivery. It provides recommendations regarding blood work and screening tests that may be completed. Our hope is the Postpartum Health Record will encourage new moms, their families and their doctors to think about, discuss, and work towards healthy lifestyle choices that reduce a mother’s risk of heart disease.
